Once the fee has been processed, we will notify you that the application checklist items are ready for you to complete.
Please see below the required checklists per division.
Early Childhood (PK3 & PK4) & Elementary School (Kindergarten to 5th grade)
- Applicant Information: citizenship, languages, current school & household demographics
- Forms: parent questionnaire, MEDUCA declaration form, family declaration form and, if applicable, learning support form.
- Homeroom teacher recommendation form. You only need to provide the email address as the system will automatically send them an email with the form to fill-in directly.
- Uploads: previous three years of school report cards, student photo, birth certificate, and, if applicable, educational testing or diagnosis and/or IEP accommodations.
Middle & High School (6th to 12th grade)
- Applicant Information: citizenship, languages, current school & household demographics
- Forms: parent questionnaire, MEDUCA declaration form, family declaration form and, if applicable, learning support form.
- English, Math & Principal/Counselor recommendation forms (3). You only need to provide the email address as the system will automatically send them an email with the form to fill-in directly.
- Uploads: previous three years of school report cards, student photo, birth certificate, and, if applicable, educational testing or diagnosis and/or IEP accommodations.
